Dementia advisers are named workers for people living with dementia. They provide ongoing assessments and provide advice, information, support, monitoring and client representation as appropriate. Advisers are the first point of contact for people with dementia, their carers and other professionals who may need advice and support. These people can be referred to outside services as required. This typically starts with a home visit to assess the needs of person with dementia and the carers. During these visits, advice and information can be provided on a wide range of subjects
Our Dementia Link Workers (DLWs) work alongside the GP Networks in Ealing and the West London Health Trust. They support people (with and without dementia medication) who have been transferred from the cognitive and dementia services with a diagnosis of dementia.
The clubs provide people with dementia a chance to socialise in a safe environment and offer respite for their carers. During the clubs carers get a break from caring and can spend the time doing whatever they would like to do.
